in which you have participated that may be relevant to the position of IPA or
that demonstrate your personal qualities.

6. Please answer the following questions regarding your ability to drive and
availability to participate as an IPA:
a) Will you have a valid Calif. driver’s license and minimum insurance
coverage (as legally required by the State of California) by the beginning of
September?
During the next academic year (Sept., early January, and Spring Break)?
b) Will you be free?
c) Do you have access to a car?
d) Are you able and willing to drive a university van to assist with
transportation?
e) If yes to a-c above, are you willing to assist with transportation using
your personal vehicle during the month of September and during the Winter break
and/or Spring quarter Orientations for the next academic year if mileage is
reimbursed?
7. Are you involved with any group, club, organization, fraternity, sorority,
job, etc. that will require much or your time during the month of September?
Please describe your time commitment to that organization?
8. Are you willing to volunteer at least 60 hours of your time during the month
of September, and 10 to 15 hours for Winter and
Spring Quarter Orientations?
